If you have ever opened a medical bill weeks after a visit and felt your stomach drop, you are not alone. More and more people are price shopping orthopaedic care for the same reason they price shop everything else that matters: they want to know what something costs before they buy it. For many patients, transparent cash pricing turns out to be clearer, faster, and often cheaper than the opaque world of insurance billing.
Why Price Shopping Orthopaedic Care Makes Sense
When you use insurance, the number you owe is rarely the number you were quoted. A single knee visit can generate a facility fee, a professional fee, an imaging charge, and a separate bill from a provider you never met. Each one runs through deductibles, coinsurance, and network rules that almost nobody can predict in advance.
Cash-based care flips that. At Journey Orthopaedic Institute (JOI), we are a 100 percent cash-based Direct Specialty Care practice, which means we tell you the price up front and you pay one clear amount. No claims, no network games, no bill arriving months later.
A few real examples of our published prices:
- In-person new patient office visit, 45 minutes: $400
- Virtual second opinion or new patient tele-visit, 30 minutes: $350
- Physical therapy initial evaluation and treatment, 60 minutes: $235
You see those numbers before you book. That is the whole point.
How Surprise Bills Actually Happen
Surprise bills are not usually the result of dishonest providers. They are a byproduct of a system with too many moving parts. Here is how a routine visit can balloon:
- The clinic bills its fee, then the hospital or facility adds its own facility fee.
- Imaging, labs, or an outside reading get billed separately, sometimes by a company you never spoke to.
- Your plan applies the charges to a deductible you did not realize was unmet, so a covered service still lands in your lap.
- A provider in the building turns out to be out of network even though the facility was in network.
By the time all of that settles, the friendly estimate at the front desk bears little resemblance to the final total. When you price shop in advance, you remove most of these unknowns before they ever start.
How to Compare All-In Prices the Smart Way
The single most useful habit when price shopping orthopaedic care is to ask for the all-in price, not the headline number. A cheap office visit is not a bargain if it triggers four downstream charges.
Use these questions every time:
- What is the total price for this visit or procedure, including the facility, the provider, and any imaging?
- If something extra is recommended, will I get the price before it happens?
- Can you give me a Good Faith Estimate in writing?
Under federal rules, cash-pay and self-pay patients are entitled to a Good Faith Estimate. It is one of the most powerful tools you have, and you should use it. At JOI most services carry a published price, orthobiologics, hyaluronic acid, and wide-awake WALANT hand surgery included. For the few that are quoted per case, such as surgery bundles, we will give you a Good Faith Estimate and tell you whether you are even a candidate before anyone commits to anything.
Where Superbills Fit In
Cash pricing does not mean you give up on your insurance entirely. After your visit, we provide a superbill, an itemized receipt with the codes your insurer needs. You can submit it to your plan for possible out-of-network reimbursement. Orthobiologics and StemWave shockwave are the exception: they are cash only. They are not covered by any insurance, so there is no claim to file and nothing arrives afterwards. The price you see is the entire transaction.
Reimbursement is never guaranteed, and it depends entirely on your plan. We will not tell you what to expect, because we cannot control it and we have seen people budget around a number that never arrived. Plans reimburse against their own allowed amount rather than against what you paid, so what comes back is often a fraction of the price, and plans with no out-of-network benefits pay nothing at all. Where your plan does have out-of-network benefits, the claim can count toward your out-of-network deductible, and we will help you file it. The difference is that you control the process: you know your cost on day one, and anything that comes back is on top of a price that was already fair.
