Common Symptoms of an ACL Tear
The signs of an ACL injury are usually hard to ignore. Watch for:
- A pop or snap at the moment of injury
- Rapid swelling within the first few hours
- A feeling that the knee is loose, unstable, or giving way
- Pain with walking, pivoting, or putting weight on the leg
- Reduced range of motion
If your knee buckles when you try to cut or turn, that instability is one of the clearest reasons to get evaluated quickly.
How an ACL Tear Is Diagnosed
Diagnosis starts with a focused history and a hands-on exam. Specific tests let a surgeon feel how much the knee shifts, which points strongly toward an ACL tear. At JOI, in-office imaging with a SMART-C mini C-arm lets us look at the knee during your visit, and we can coordinate advanced imaging such as MRI to confirm the tear and check for related meniscus or cartilage damage.

ACL Tear Treatment: Rehab or Reconstruction
Here is the honest part. The real choice in ACL tear treatment is usually between structured non-surgical rehab and surgical reconstruction, and the right answer depends on you.
Non-surgical rehab can be a strong option for people who are lower-demand, who do not need to cut and pivot at high levels, or who respond well to focused strengthening. The goal is a stable, confident knee built through guided physical therapy.
Reconstruction is often the better path for athletes and active people who need to return to cutting and pivoting sports, or for knees that keep giving way despite rehab. Surgery rebuilds the ligament so the knee can handle rotational load again.
A surgeon-led evaluation separates a guess from a plan. We will tell you plainly when surgery is needed and, just as important, when it is not.
Where Physical Therapy and Biologics Fit
Physical therapy is central no matter which path you choose. Before surgery, prehab calms swelling and restores motion and strength, which improves results afterward. After surgery, structured PT gets you back to confident, full activity. At JOI, integrated physical therapy is led by Dr. Christie Booth Smith, PT, DPT, KEOMPT.
Public PT pricing is straightforward: an initial PT evaluation and treatment (60 min) is $235, a subsequent PT session (45 min) is $195, and an evaluation plus 5 sessions package is $1,000.
Orthobiologics such as PRP, BMAC, and Lipogems may have a role for certain knee injuries and partial damage, often alongside rehab. Whether you are a candidate depends on your imaging and goals. Biologic prices are published: PRP is $750 per site, BMAC is $2,500, and Lipogems is $5,500, cash only. Ask whether they fit your case.
The JOI Clear-Price Path for ACL Care
JOI is a 100 percent cash-based, out-of-network Direct Specialty Care practice. That means transparent, upfront pricing with no surprise bills. If your plan has out-of-network benefits, we provide an itemized superbill and can file the claim on your behalf, though there is no guarantee of reimbursement. Orthobiologics are the exception: PRP, BMAC, and Lipogems are cash only. They are not covered by any insurance, so there is no claim to file and nothing arrives afterwards. The price you see is the entire transaction.
An in-person new patient office visit (45 min) is $400, and a virtual second opinion or new patient tele-visit (30 min) is $350. If you prefer to be seen where you are, a mobile medical unit consult (45 min) is $500.
